Job Description

Job Title:                     Social Services Specialist

Department:              Social Services and/or Administration 

Reports To:                 Nursing Home Administrator 

 

POSITION PURPOSE:

Welcomes families and performs Case Manager duties for all Skilled Residents including grievances; coordinates intake admission and discharge procedures.

 

NATURE & SCOPE:

 

?The Social Services Specialist (SSP) reports to the Nursing Home Administrator

?The Social Services Specialist (SSP) chairs the 3 Day Admission Meeting (i.e., Initial Evaluations)

?Monitors Episode Connect (Remedy Partners) for pending admissions & communications of skilled services

while here at facility;

?Handles POA’s, Advance Directives, and PASRR’s for all skilled residents;

?Assists with resources regarding hearing aids, glasses, dentures & follows-ups for all skilled residents; ?Completes all MDS and Care Plans for skilled residents;

?Plans and follow through with all discharge planning for skilled residents:

1. DME equipment,

2. Home Health Set-up
